Appeal Forms

Begin the academic appeal process by signing up for an appointment to get assistance with completing both the Dismissal Appeal and Readmission Form and the Academic Success Action Plan (ASAP). You will complete your ASAP after meeting with your CAE Academic Success Coach or TRIO SSS Advisor.

• To access the form:

1. 1. log into luther.pharos360.com and
   2. Input your Luther Username and Password if prompted to.
   3. Select ‘Ask for Help’ and select the ‘Academic Success Action Plan (ASAP)’ form.
