2018 Wellness Screening Overview

Luther College is committed to promoting the safety, health and wellbeing of all of its employees and their families.  To support that commitment every even year the Nena Amundson Lifetime Wellness Program and Human Resources partner to promote employee wellness screenings.

Each even year these screenings help individuals to identify health conditions such as pre-diabetes, cancer, high blood pressure, high cholesterol, and more that otherwise may go undetected.  This helps the individual to get early preventative care, guidance, gain years of a happy-healthy life, and reduce spending on major future care that may have been needed.

All employees and spouses covered by the Luther College Health Care Plan who complete the following four steps by December 31st, 2018 will avoid an additional monthly health care premium of $25.00, which otherwise will begin on January 1, 2019.

1. Complete a Community Wellness blood panel.

2. Fill out a clinical health risk assessment (CHRA).

3. Complete a wellness check-up physical exam with your primary care provider.

4. Fill out the Wellness Check-up Form and turn it in to Human Resources.

 Detailed Instructions and Troubleshooting

1. Community Wellness Blood Panels

Community Wellness Blood Panels may be completed at either Winneshiek Medical Center (WMC), Decorah Gundersen Clinic, or another in-network care provider.  The wellness program has also partnered with WMC and Gundersen to bring screenings to campus every month (see dates below).  Please email Vicky Jaeger to schedule a time.

  • Gundersen Dates: 1/16, 3/14, 5/15, 9/12, 10/16, 12/12 all from 6:30-8:00 a.m.
  • WMC Dates: 2/6, 4/10, 6/5, 8/28, 11/13, 12/11 all from 6:30-8:00 a.m.

Community Wellness Blood Panels cost $40-45, which is reimbursed by insurance, and measure your Lipids panel (cholesterol, triglycerides, HDL and LDL) , glucose (blood sugar), TSH, hemoglobin, creatinine, potassium, calcium, ALT, and sodium.  Additional testing for PSA and Hemoglobin A1C may be available.  If you feel you may benefit from one of these two additional tests an order from your primary care physician may be required, and insurance coverage varies. If you are unsure if you would benefit from these tests, we advise discussing them with your primary care provider first.

You must fast for 10-12 hours before your panel for accurate results. This means consuming no food or beverage, except water.

Unless you need a physician's order for an additional blood screening we recommend that you complete the Community Wellness Blood Panel first, so that you can record the results on your CHRA and discuss them with your primary care provider at your wellness physical exam.

If completing your Community Wellness Blood panel by walking into a clinic you must pay $45 at the time of your blood draw, collect a receipt, and submit it for reimbursement. If you complete the panel on campus we have negotiated a reduced rate of $40, which you may pay by cash or check to Luther College at the time of your blood draw, and Luther will submit the reimbursement for you automatically.

The results of your panel are strictly confidential, and protected by HIPPA.  Luther College does not have access to your results. On campus screening results will be delivered to you via campus mail in a sealed envelope.

2. Clinical Health Risk Assessment (CHRAs)

CHRAs are completed online on the UMR portal websitePlease note, access to the CHRA is not yet available and will open later in January. If you would prefer to complete a paper copy of the CHRA contact Vicky Jaeger, and you will be provided with one and a pre-addressed envelope to confidentially send it to UMR.

The process is the same for those who participated in 2016. Simple log into your account, or if you forgot your password you can click on the "forgot username or password?" link. New members can click on "New User. Register here." and use their insurance id card to get registered. 

Please note that if you have a spouse on the Luther Health Care Plan they have their own log in to the site, or must create one.  Each of you must log in to your own accounts to complete your CHRA.  If you are experiencing difficulties logging out of one account and into the other be sure that you have entirely logged out of your session by closing your browser.  You may need to also clear your cache and cookies.

The information you provide is strictly confidential, and protected by HIPPA.  Luther College does not have access to your individual CHRA. The Wellness Program staff only receives a summary of all employees/spouses data combined, which is used to plan programming and wellness opportunities to support the health and wellbeing of our employees and their families.

We recommend that you complete the CHRA second, after your Community Wellness Blood Panel, so that the information from your blood results can be entered in the CHRA.  You can then take both of these to your Wellness Check-up Physical Exam to discuss them with your primary care provider.

3. Wellness Check-up Physical Exam

A Wellness Check-up Physical Exam will allow you to discuss the results of your Community Wellness Blood Panel and CHRA with your primary care provider. 

Simply schedule an appointment with your primary care provider at your earliest convenience.  Be sure to request an annual "Wellness Check-up" and to schedule it early in the year to ensure it is completed by the end of the calendar year. The check up is covered 100% by insurance. Please note that your provider may request additional tests or procedures based on your check-up, health conditions, blood panel results, or CHRA that may not be covered by insurance. Please be sure to contact UMR or HR with questions about coverage.

The results of your appointment are strictly confidential, and protected by HIPPA.  Luther College does not have access to medical records.

4. Wellness Check-up Form

After completing the above 3 steps please fill out a Wellness Check-up Form and turn it in to Human Resources by December 31st, 2018. If your spouses is covered by the Luther College Health Care Plan they must complete a separate form.
