A student may request disability services in two ways. First, the student may complete the Request for Disability Services form shown below, or the student may simply contact the Disability Services Office (DSO) and schedule an interview.  Regardless of which method is chosen, both require completion of the Documentation of Disability form listed below.  This form needs to be completed by the professional who diagnosed the disability.  Refer to the Luther Process.
