Hardware Requests

This form is designed for faculty and staff to request non-standard hardware. Standard hardware includes a workstation, keyboard, mouse, and monitor (for desktops). To request hardware, please complete this form and submit it electronically by selecting “submit”. Include as much information as possible. Your request will be processed by LIS and you will be contacted within 5-7 business days.
