There is no health program benefit for the following:

• pre-existing condition prior to the date of enrollment
• medications
• consultation, examination, laboratory tests for contraception, prenatal care, abortion, or complications therefrom
• eye examination, eyeglasses, and similar appliances
• dental or periodontal treatment
• ambulance
• hospital emergency room services
• consultation services not provided on campus
• laboratory or x-ray procedures not done on campus
• elective surgeries or treatment
• accidental injury involving self-propelled vehicle motor vehicle, airplane, boat, snowmobile, jet ski
• injuries sustained while scuba diving, flying, sky diving, ballooning, bungee jumping, or hang gliding
• losses resulting from war or any act of war (declared or undeclared) while in the armed forces for any country
• treatment in a federal hospital or any expense payable under worker's compensation or similar legislation
